The case manager helps identify appropriate providers and facilities throughout the continuum of services, while ensuring that available resources are being used in a timely and cost-effective manner in order to obtain optimum value for both the client and the reimbursement source.

Patient navigators guide patients through the health care system and overcome barriers that prevent them from getting the care they need. Patient navigation helps people get the health care and other resources they need to be as healthy as possible.

Job Description The System Navigator is available to survivors before, during and after to assist in accessing resources, reconnecting with the Police Task Force and to provide updates on the work of the Task Force. They work closely with the Human Rights Monitor, Cultural Monitors and Police Task Force.

You plan care goals for patients, support and educate them about treatment options, help them understand referrals, assist with admissions and discharge paperwork, and advocate for them to ensure they receive the highest quality care.

In short, a patient or nurse navigator is your advocate for care from the initial consultation right through treatment. Nurse navigators are most commonly found in oncology to help patients through their cancer diagnosis and individualized care plan.

They are trained to work with all the resources available to them which may include signposting patients to another service, offering self help advice or booking with an appropriate clinician in the practices multi-disciplinary team. This work is often referred to as care navigation.
